PREAMBLE

 These bylaws are adopted in order to provide for the organization of the medical staff of Citrus Valley Medical Center (Inter-Community Campus/Queen of the Valley Campus) and to provide a framework for self-government in order to permit the medical staff to discharge its responsibilities in matters involving the quality of medical care, and to govern the orderly resolution of those purposes. These bylaws provide the professional and legal structure for medical staff operations, organized medical staff relations with the Board of Directors, and relations with applicants to and members of the medical staff.

 

DEFINITIONS

 1. ADMINISTRATOR means the person appointed by the Board of Directors to serve in an administrative capacity.

 2.  AUTHORIZED REPRESENTATIVE or HOSPITAL'S AUTHORIZED REPRESENTATIVE means the individual designated by the hospital and approved by the Medical Executive Committee to provide information to and request information from the National Practitioner Data Bank according to the terms of these bylaws.

 3.  BOARD OF DIRECTORS means the governing body of the hospital.

 4.  CHIEF OF STAFF/PRESIDENT means the chief officer of the medical staff elected by members of the medical staff.

 5.  PRIVILEGES means the permission granted to medical staff members to provide patient care and includes reasonable access to those hospital resources (including equipment, facilities and hospital personnel) which are necessary to effectively exercise those privileges. Such privileges may be restricted to a campus pursuant to an exclusive contract.

 6.  HOSPITAL means Citrus Valley Medical Center (Inter-Community Campus/Queen of the Valley Campus.)

 7.  INVESTIGATION means a formal process specifically instigated by the Medical Executive Committee to determine the validity, if any, to a concern or complaint raised against a member of the medical staff, and does not include activity of the Physicians' Advisory Committee.

 8.  MEDICAL EXECUTIVE COMMITTEE means the executive committee of the medical staff, which constitutes the governing body of the medical staff as described in these bylaws.

 9.  MEDICAL STAFF or STAFF means those physicians (MD or DO or their equivalent as defined in Section 2.2-2(a)), dentists or podiatrists who have been granted recognition as members of the medical staff pursuant to the terms of these bylaws.

 10.  MEDICAL STAFF YEAR means the period from January 1 to December 31.

 11.  MEMBER means, unless otherwise expressly limited, any physician (MD or DO or their equivalent as defined in Section 2.2-2(a)), dentist or podiatrist holding a current license to practice within the scope of that license who is a member of the medical staff.

 12.  PHYSICIAN means an individual with a MD or DO degree or their equivalent. "Their equivalent" shall mean any degree (i.e., foreign) recognized by the licensing boards in the State of California to practice medicine.

 13.  CAMPUS means Inter-Community Campus 210 W. San Bernardino Road, Covina, CA 91723 and Queen of the Valley Campus 1115 So. Sunset Avenue, West Covina, CA 91790.
